Affiliate Forms

Annual Entry Form
The Annual Entry Form provides The Groundwater Foundation with information about your entity, including your team contact, organizational mission, service area, and areas of assistance offered.

Team List
Each entity must form a team that includes at least four members. Please include all contact information for each member.

Result Oriented Service (ROS) Plan
The ROS plan form functions as a planning worksheet for Affiliate teams as they develop and implement local services. An ROS plan should be prepared and submitted for each individual activity the team decides to take on.

Annual Report
Designations are awarded by a subcommittee of the Groundwater Guardian Council based on the information provided in the Annual Report. The report includes a summary of the services implemented by an entity during the program year.
