Affiliate Forms
An entity must submit the following forms to be considered for designation as a Groundwater Guardian Affiliate. Click on the name of the form to download. Please contact The Groundwater Foundation at 1-800-858-4844 or guardian@groundwater.org if you have any questions regarding these forms. Forms can be submitted by e-mail to guardian@groundwater.org, mail to The Groundwater Foundation (P.O. Box 22558, Lincoln, NE 68542-2558), or fax to 402-434-2742.
Groundwater Guardian Affiliate Anual Entry Package
The Annual Entry Package includes the Annual Entry Form, Team List, and Result Oriented Service Plans. The package provides The Groundwater Foundation with information about each organization, its team, and its services.

New Affiliates may enter at any time using the Annual Entry Package. Continuing Affiliates must resubmit the Annual Entry Form, Team List, and Result Oriented Service Plan by February 15 of each program year.
Annual Entry Form
The Annual Entry Form provides The Groundwater Foundation with information about an organization, including team contact, organizational mission, service area, and areas of assistance offered.

Team List
Each entity must form a team that includes at least four members. Please include all contact information for each member.

Result Oriented Service (ROS) Plan
The ROS plan form functions as a planning worksheet for Affiliate teams as they develop and implement local services. An ROS plan should be prepared and submitted for each individual activity the team decides to take on.

Annual Progress Report
Designations are awarded by a subcommittee of the Groundwater Guardian Council based on the information provided in the Annual Progress Report. The report includes a summary of the services implemented by an entity during the program year.

Annual Progress Reports are due August 31 for all Affiliates.
