Groundwater Guardian Community Forms
Teams must submit the following forms to be considered for designation as a Groundwater Guardian. Click on the name of the form to download. Groundwater Guardian Community Annual Entry Package
The Annual Entry Package includes the Annual Entry Form, Team List, and Result Oriented Activity Plans. The package provides The Groundwater Foundation with information about a community, its Groundwater Guardian team, and its groundwater education and protection activities.
Annual Entry Form
The Annual Entry Form provides The Groundwater Foundation with information about a community, including your team contact, local water supplies, groundwater-related concerns, and media information.
Team List
Each Community must form a team and list each team member and representative category on the Team List. Please include all contact information for each member.
Result Oriented Activity (ROA) Plan
The ROA plan form functions as a planning worksheet for teams as they develop and implement local activities. An ROA plan should be prepared and submitted for each individual activity.
Annual Progress Report
Designations are awarded by a subcommittee of the Groundwater Guardian Council based on the information provided in the Annual Progress Report. The report includes a summary of the activities implemented by a team during the program year.
